Mortgage Checklist
Please bring the following items to your mortgage planning session:
 
Employment/Income
  • Names, addresses, phone numbers and dates of employment for all employers in the last 2 years
  • Most recent 30 days pay stubs
  • Most recent 2 years W-2 forms
  • Most recent 2 years signed Federal Tax Returns (all schedules). If you do not have them call 800.829.1040 to request tax transcripts from the IRS
  • If self-employed, last 2 year’s signed personal and corporate tax returns (if applicable) and year-to-date profit and loss statement and balance sheet
  • Social Security or Retirement awards letter and previous 2 years 1099 forms, if applicable
  • If child support is received, divorce decree & settlement agreement/child support court order along with a printout from court, cancelled checks or bank statements showing on time receipt for past 12 months
  • If a full-time student in the past 2 years, college transcripts (can be printed from internet)
Asset Information 
  • Most recent 2 months bank statements. Please provide all pages to the statement (ex: if statement shows page 1 of 6, 2 of 6, etc. provide all thru page 6 of 6 even if some pages are blank)
  • Most recent monthly/quarterly statement for mutual funds, 401k, other retirement accounts or other assets. Also provide evidence that you have the ability to withdraw funds from these accounts. (Most of the time you can pull this information from the account provider’s internet site.)
  • Provide a letter of explanation and supporting documentation regarding any large deposits (other than paychecks) that are over $200.  
  • If home was recently sold, copy of HUD-1 Settlement Statement from closing
  • Listing agreement on current home, if applicable
  • If you own a rental property, address, mortgage information and signed lease agreement.
 Additional Information
  • Bring driver’s valid license with you and we will make a photocopy
  • Completed and Signed Credit Inquiry Letter (Click Here for a Sample)
  • If purchasing a home, provide a copy of your cancelled earnest money check. Click Here for a Sample.
  • Previous 2 years’ residency history and landlord contact information
  • If bankrupt in past 10 years:
    • Complete file-stamped bankruptcy papers w/ all schedules and discharge letter
    • Detailed letter of explanation for the bankruptcy
    • If you filed in Indiana and do not have your bankruptcy papers please call 317.229.3800
  • If divorced, complete file-stamped divorce decree and settlement agreement
  • If applying for a VA Mortgage, Certificate of Eligibility and DD214 form
  • If child support is paid, divorce decree & settlement agreement/court order with proof of payments for past 12 months
  • If applying for an Indiana Housing programs (Bond or MCC), past 3 years sign Federal Tax Returns
  • If you will be saving funds for down payment, please save all bank statements and pay stubs.
